Expression of Interest Honour Roll Wall - SWHT
The Honour Roll Wall is located at Submariners' Walk Heritage Trail, Teneriffe, Queensland. Any
former submariner, regardless of nationality, can have a bronze plaque installed on the
Eternal Patrol Wall.The Submarines Association Australia Qld Inc (SAA QLD Inc) has delegated a sub-committee whose responsibility is to investigate the eligibility of all applications. Once verification is confirmed to be legitimate, a representative of the sub-committee will contact you, as the applicant, to begin the formal procedures for a bronze plaque to be manufactured and affixed to the “Wall”.

How to Apply
1. Email the sub-committee directly via the email address swhthonourwall@gmail.com and one of the committee members will make contact and send the application form as well as other documentation outlining the process.
2. It is essential to be able to provide proof of submarine service .This can be from official records, or Statutory Declaration from another submariner of good standing, OR relevant documentation to prove submariner status e.g.
Dr Michael White OAM book - Australian Submarines - A History. This must be submitted with the completed application form.
3. It is important to note that all documentation submitted must be certified, dated, signed and witnessed by a Justice of the Peace, Solicitor, Barrister or Commissioner for Declarations.
Please allow up to 8 weeks for the art work, manufacturing and installation process.

As a result of the strict Queensland Heritage and Brisbane City Councils criteria the wording on the bronze plaque is limited to the submariners Surname, Initials, Rank, Service No (if applicable), Honour and Awards (if applicable) Country of service, Date of Birth and Date of Death.
